Please complete and submit the required information below to initiate a payment claim under the Surface+ Offer for Microsoft Customers. Upon submission, your Microsoft account manager and account team will be notified. Your company contact will also receive a confirmation email.
You will receive a confirmation of your claim status from the Surface+ Offer support team within 14 days of your claim submission.
Important: Please review the Surface+ Offer for Microsoft Customers eligibility criteria in the Terms and Conditions before submitting your claim. You may upload multiple Proof of Execution files at one time; please upload all proof of execution files before submitting. Proof of execution must include your company name, reseller name, invoice date, device description and/or SKU, and quantity purchase. We strongly recommend removing any additional details, including pricing information, bank names, account and/or routing numbers, and SWIFT codes.
Submit your claim only once; please wait for the confirmation page to load after clicking the Submit Claim button.
